Through a rating system that estimates the number of viewers statistically. It's not an exact count, nor is an exact count necessary for any purpose. We're not quite like the more German-style culture, like the Dutch, who are so regimented and demanding of excessive order that the notion of not being counted, lined up, categorized, and placed in appropriate bins seems alien to you.
This does present a big problem on the immigration side, as we are grudgingly being dragged into the practice of "national ID card" to pigeonhole everyone, make sure their tracked and categorized, and appropriately lined up. Don't worry, we'll get there. You'll be happy that we're all recorded and enumerated soon enough.
