CNN: Nirmal Mulye, founder and president of Nostrum Pharmaceuticals, commented in a story Tuesday about the decision to raise the price of an antibiotic mixture called nitrofurantoin from about $500 per bottle to more than $2,300. The drug is listed by the World Health Organization as an "essential" medicine for lower urinary tract infections.
"I think it is a moral requirement to make money when you can," Mulye told the Financial Times, "to sell the product for the highest price."

that employee and another witness maybe, signs papers of the batch. A supervisor reviews. It may or may not be tested as to what the formulation came out.

This is a drug mixing operation.

Active ingredients are mixed with other components until uniform. The mix goes to tableting. Extensive tests on the tablets are done before the batch is released. There are batch records for all steps until the batch is in boxes to go to pharmacies. The packages themselves have FDA rules, labels and inserted booklets etc.

Intrigued, I did a bit of research. The issue of compounding pharmacies in the Netherlands was in the news rather recently.

'Dutch join backlash at expensive drugs by making their own'
Pharmacies retain the right under European and American law to prepare medicines for individual patients. Common examples including making lower dosage versions for children or liquid versions for people with difficulty swallowing.

However, in the United States, for-profit compounding pharmacies have tested the limits of what they are allowed to do in recent years, including mixing medicines in large quantities. In some cases, that has prompted legal conflicts with drugmakers.

Official scrutiny of the practice increased after the 2012 deaths in Massachusetts, which led to U.S. lawmakers requiring bulk compounding facilities to register with the Food and Drug Administration (FDA) and meet quality and labeling standards - still not as stringent as full FDA drug approval.

The general question of why drugs are expensive, or how they can charge the rates has 4 answers

1 they have to be invented
You have to recruit inventors at 100 000 or more annual salary
Some 20 inventors working 10 years will produce one drug that passes drug trials.

2 patents

3 small scale product takes as much labor as one that is 5-10 times bigger in scale and has 10 times more patients

4 if you are the only maker, it will take 2 years for another outfit to get an FDA approved drug running

Companies manufacture drugs. Compounding pharmacies compound medications.

Well, they buy from the same big pharma, India or China. They then formulate it. They do not make the active ingredient. The Italian outfit posted by mecis more of a research lab and goes to drug truals the same way as big pharma does.

Let us say those 20 inventors (more likely 40 as there is a biologist working with each PhD) get 20 drugs to hymanntrials in 10 years. Some 19 will fail the very expensive human trials. Either they work as well as the previous best (but not better) or they fail on side effects. Well, your winner, 1/20, will have to pay the cost of the hospitaltrials of those 19 that failed.

Trump's evil drug czar is right:
"Canada simply doesn't have enough drugs to sell them to us for less money, and drug companies won't sell Canada or Europe more just to have them imported here," Azar said in remarks about the President's blue print to lower drug prices last May.
Also, he added, the last four commissioners of the Food and Drug Administration have said there's no way to effective ensure that the medication is really coming from Canada, and not a counterfeit factory in China.
