It would be helpful to knock out Russian comms and radar first. Probably not as difficult a task as you'd think. ICBMs are super rapid and can be fitted with conventional warheads, you can eliminate a Russian silo with a US-launched missile in under half an hour. Less if you launch from a sub.

Russia's nuclear subs are a separate problem but the US and UK would be pretty confident of knowing exactly where they all are and taking them out before they can launch.

The biggest challenge would be Russia's mobile missile units that run on rails. Although important to note that the US (and by extension, NATO) has impressive ABM systems.

tldr - I am not overly worried about Russia launching nukes, and nobody else should be

I'm wondering what the Islamic State motivation is for such an attack in Russia. Perhaps connected to Chechnya? Perhaps just a hatred of the West? (though including Russia in the "West" is stretching it...)

I think the appropriate response would be to split Russia into two states between Russia and Islamic State, with IS given the eastern half of Moscow as its capital.
