I don't know what basic Medicare covers. Everyone I know buys a supplemental insurance plan. Mine costs $150 a month, which while not free, is a fuck of a lot cheaper than the income tax rates I paid in the UK or what I see y'all pay in Oz. It comes out of my Social Security check so I never see it. Teh Gummint gives with one hand and takes with the other. I pay 0% deductible (called "excess" in some markets) and $0 co-payments, with no lifetime limits. So far so good, my out of pocket expenses have been 0. I'm sure there are some things I would get a bill for, but it's the best medical coverage I've ever had. I pay another $19 a month for a "Platinum Level" prescription plan, so my medications (the usual trinity of cholesterol, heart, and thyroid) are "free". Previously I was paying $45 a month.

That said it's an enormously expensive, wasteful, regulation and bureaucracy ridden program, but I'm beyond caring. As long as it takes care of me I don't give a shit, it's not like I can affect their policies anyway so I just take what's on offer.

The biggets downside is it has very limited coverage overseas, a $50,000 lifetime limit. That's a lot of medical care in Swaziland or South Africa, though, and I'll buy some sort of policy there.

What I like about the system in Oz is that medicare costs are progressive. Last time I checked the medicare levy for most tax payers is an additional 2% on their taxable income. So, someone on a taxable income of, say $22k will pay an additional $440 on their annual income tax, while someone on a taxable income of $90,000 pays an additional $1800.

That's an oversimplified picture, though. The situation is rather more complex. There also are exemptions and surcharges. People who earn $21,980 or less, (or $34,758 for seniors and pensioners entitled to the seniors and pensioners tax offset) are totally exempt from the medicare levy, while those who earn more than $90,000 get lumbered with an additional 1% unless they also have a private health insurance policy. The scales are also adjusted according to whether you are part of a family and how many children there are in that family.

Our healthcare system is nowhere even near perfect, but I very much prefer it to yours.

The Biden plan is to cap insurance at some 8% of your income. So the top earners would pay that. It kind of skews things.

Bear in mind that the Medicare for All bills from Bernie and Jayapal are free and comprehensive, and also include eye and dental. They're far more than just Medicare expanded to include everyone. No copays, no deductibles, no premiums...

But you are right, they have to package it right. We will end up with Medicare providing basic healthcare (including childbirth) and the emergency room. Supplements providing the rest. The employers will have an easyvtransition to these supplements for the employees. And since they too will be regulated, even small time companies (50 or so) can provide this benefit.
