Trump was asking second amendment supporters to vote for him, not to shoot hillary. Just as he was warning his supporters that the democrats want easy untraceable voter fraud which is why he wants voter id.

He's not insane - he's idiosyncratic. Diagnosing mental disorders in those one disagrees with, based on little more than one's disagreement with them, is unhelpful as political commentary and disrespectful to those with a genuine mental health diagnosis.

He said it in the context of what happens if she gets elected. The context was clear, it was not about voting to stop her from getting elected, it was about what could be done once she was elected in order to stop her from appointing justices. Either his language is accidentally confusing and ill thought out, or he knows exactly what he is implying (he does) but that when done in a certain way he can get away with it (because he has so many times). Even if the former is true, that still speaks to incompetence.
